  • This form is used to withdraw from all classes within a current term and/or from the university.
  • **Withdrawal Form Statement: A student may withdraw from school without academic penalty through the date identified in the Academic Calendar (https://www.faulkner.edu/academic-resources/academic-calendar/) of each program as the “last day to drop a class with a “W”. A student withdrawing from school (all courses) must complete this official withdrawal form. Your withdrawal is not official until you, the student submit the completed online form to the Registrar’s Office for processing. All withdrawals after the time noted above will result in a grade of “F” (withdrawn-failing), which will adversely affect the grade point average. You will be charged 50% or 100% of tuition based on when you receive a W/F/FA.

The OFFICIAL date of withdrawal is determined by the date that the first contact is made by the student to initiate the withdrawal process. Charges and/or refunds to the student’s account and grades are based on this date.
