CPT Application Step 1 of 2 50% Instructions to complete the CPT ApplicationThis is a type of off-campus work/internship that is directly tied to the student’s program of study. It is not available after the student completes their respective degree program.On the next page, you will begin the CPT application process by filling out your basic information and clicking “submit”. Upon submission, you will receive two emails from Faulkner University: 1.) One will request additional documentation needed to complete your CPT application. 2.) The second email will contain information about the Cooperative Agreement you will send to your employer. Please check your spam/junk email folder for an email from donotreply@faulkner.edu. If you do not receive these emails, contact CPT@faulkner.edu if you have any questions. Cooperative Agreement You will receive the Cooperative Agreement after you submit this form. The cooperative agreement will need to be sent to the employer by the student using their preferred method. The agreement must then be completed and signed by the employer or the CPT application will be denied. Typed script font computer signatures will not be accepted. The signature must be hand-signed by the employer or an electronic signature through a trusted, verified site such as DocuSign by the employer. Once completed, the agreement must be sent to CPT@faulkner.edu. After completing this formThe following documents are required for the completion of the CPT application process. Please have all of these documents ready to send via email before moving on to the next page of this form. Failure to submit these items will result in your CPT application being denied. Students who are required to resubmit an application will go back into the queue for processing. 1.) Offer Letter Must include work site location and job title. Must be dated within the past six (6) months or CPT application will be denied. Must be sent as a non-editable PDF document. Editable documents (such as Word Doc) will not be accepted. 2.) Job Description Provide a very detailed job description listing roles and responsibilities specific to your position on company letterhead (if not already specified in the offer letter). 3.)
